The package itself is pretty simple, the comment section is rendered by some Facebook's javascript and the package don't have much control over it.
Possible explanations would be:
1) Image is actually coming from facebook, it may be a cached version of an old avatar - clearing browser's cache should solve the problem
2) Image coming for some other kind of source (Gravatar ?). Check what the image url is in FireBug or Chrome's developer toolbar (active by pressing F12)
